Join our team at Highland Park Rehabilitation and Healthcare Center as a Licensed Social Worker.Proudly supported by Marquis Health Consulting Services$70,000-$90,000/yearResponsibilities of Licensed Social Worker:Evaluate patients (existing, new and prospective) based on their needs and limitationsAddress patient concerns and goals while maintaining constant communication with the patient as well as a case managerBuild rapport with patients and their familiesCoordinate care with PT departmentsDevelop care plans for patientsQualifications of Licensed Social Worker:Primary series of the COVID-19 vaccine requiredMust be able to work full-time hours and be flexibleBachelors degree required. Masters degree is strongly preferred.Minimum three (1) years' experience as a social worker in a healthcare-related setting.
