Job Title: RN Case Manager Location: Bradford, MA 01835 Pay
Rate: Up to $2519/week Start Date: 07/28/2025 Job Type: Contract

facilities. Position Summary: Must have MA license in hand for
acceptance 1-year plus experience in Case Management, LTAC/Rehab CM
experience must be specific for hospital discharge planning
(rehabilitation hospital experience a plus but not required)
Familiar with EMR and/or documentation expectations for role (ACMA
SOP) Familiar with Continuity of Care / Transition of Care
requirements (CMS standards ) Shift: Mon-Friday 8-5p Shift is 5x8
days but any variation to 8 hour shift (staying late/coming in
early) has to be discussed and approved by manager Key
Responsibilities: Assessing new patients by gathering information,
reviewing diagnoses and analyzing medical test results Developing
complete plans of care to address the needs of patients Educating
patients about their care plan and health care options and helping
them make key decisions Scheduling appointments for doctors’
visits, therapies and other medical services Revising care plans
based on patients’ progress and input from physicians and other
medical providers Updating patient medical records after each
contact Offering emotional support for patients and their families
Required Qualifications: 1 year of RN Case Manager, LTAC/Rehab
experience BLS Certification by AHA MA State RN License Application
