Job Description Certification Details - BLS - ASCP MLT or equivalent registration (AMT, HEW) Job Details - Generalist with blood bank and limited microbiology experience. - 4–10-hour shifts, 1p-11:30p, Tuesday – Friday. - Shared housing provided at no cost. Job Requirements - Associates’ level ASCP certification (e.g., MLT) OR equivalent registration (AMT, HEW) required. - Primary Source Verification of professional license and/or certification, as required for the position, within 30 days of start date and updated upon expiration. - Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test. - Vaccine required during flu season Sept 15th - April 30th; no exemptions or waivers considered. - Documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or documentation of a negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break. - Physician Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break. - MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break. - MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break. - MGB Resume including past 7 years of work experience with city & state. - MGB Skills Checklist required for submission. - MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions upon acceptance of assignment. - MGB Education Verification through background check company, verifying highest level education achieved. - MGB SSN Trace must be run with Criminal check and list addresses/aliases for past 7 years. - MGB National Sex Offender Check ran within 30 days of start and updated annually. - MGB Exclusion Checks ran within 30 days of start and annually, including OIG, GSA, MassHealth & NH Medicaid exclusion lists and upload attestation. Schedule Information - 4–10-hour shifts, 1p-11:30p, Tuesday – Friday. Additional Details - Shared housing provided at no cost. - MGB Nantucket Housing Agreement required upon hire if staying in NCH housing. - MGB FEMA ICS 100 Training must be completed, no exceptions, does not expire.
