Job Description:
Aveanna Healthcare, a national leader in home healthcare
services, is seeking a dedicated Home Health Physical Therapy
Assistant to join our team. This position offers the opportunity to
work closely with pediatric and adult patients within their homes,
employing cutting-edge therapies to enhance daily living and
physical capabilities. A successful candidate will assist in the
implementation of physical therapy plans that help patients achieve
meaningful progress. This role requires travel to patients' homes
with a focus on compassionate, effective treatment planning and
execution. Assist physical therapists in the evaluation of patients
and the implementation of prescribed therapy plans. Provide therapy
services under the supervision of a licensed physical therapist,
focusing on mobility, strengthening, and coordination. Record
patient progress and adjust treatments as necessary, ensuring
timely communication with the supervising therapist. Educate
patients and their families on therapeutic exercises and home
safety and rehabilitation techniques. Maintain accurate clinical
records and comply with all legal and professional requirements.
Transport therapy equipment to patient homes and set up as needed
for effective treatment. Proficiency in implementing physical
therapy treatments tailored to patient needs. Strong interpersonal
and communication skills for effective patient and family
interaction. Ability to navigate diverse home environments and
adaptability to varying patient situations. Skills in documentation
and patient progress reporting. Medical Insurance Dental Insurance
Vision Insurance Life Insurance Valid Physical Therapy Assistant
State License. CPR certification required. Valid driver’s license
and reliable transportation, as this role requires travel to
patient homes.
