Pay: $2,720 to $2,880 per week Shift Information: Days - 5 days x 8
hours Contract Duration: 13 Weeks Start Date: ASAP About the
Position Pay Rate: $68.00 - $72.00/Hourly. Summary: The Physical
Therapist partners with residents and other care providers to
provide physical therapy services to maximize residents’ functional
abilities and independence in their activities of daily living.
Responsibilities: Evaluates residents per department protocols and
establishes a therapeutic plan of care which includes functional
long- and short-term goals, frequency and duration of services, and
therapeutic modalities to be used. Seeks input from the resident
and primary caregivers regarding the therapeutic plan of care.
Incorporates this information into the therapy goals. Provides
consultation to the referring physician and other members of the
healthcare team regarding the plan of care and discharge needs of
the resident. Attends all pertinent team meetings. Monitors and
evaluates the treatment effect and progresses the treatment program
toward the functional goals established in the treatment plan of
care. Modifies the treatment plan of care as necessary. Documents
in the Electronic Medical Record all evaluative and therapeutic
interactions with the resident per department procedures. Ensures
that the documentation is accessible to others on the care team
including ensuring language that can be easily understood.
Recommends and obtains appropriate assistive devices to the
resident ensuring that instruction has been given in their use.
Provide appropriate education to the resident and/or caregiver.
Documents the results of teaching. Refers residents appropriately
to Physical Therapist Assistant. Works collaboratively to ensure
good resident care with other therapy professionals. Requirements
Requirements: Bachelor’s degree required. Masters or Doctorate
preferred. BLS Certification, Prior experience in a long-term care
or skilled nursing facility preferred, Physical Therapy License.
