NPORT is currently seeking a dedicated and compassionate Physical Therapist (PT) to join our team in a skilled nursing and rehab center located in Amityville, NY. The ideal candidate will provide high-quality, patient-centered physical therapy services designed to improve mobility, restore function, and enhance quality of life for residents. Key Responsibilities: Evaluate residents to determine functional abilities and establish individualized treatment plans. Deliver physical therapy interventions in accordance with physician order and established care plans Provide education to patients, families, and caregivers on therapy goals, home exercises, and safety techniques. Collaborate with an interdisciplinary team (nursing, OT, speech therapy, etc.) to ensure coordinated patient care. Accurately document evaluations, progress notes, and discharge summaries in compliance with facility and regulatory standards. Participate in care plan meetings and contribute to quality improvement initiatives. Qualifications : Education: Graduate of an accredited physical therapy program Licensure: Current and valid New York State Physical Therapist license. Experience : Previous experience in a skilled nursing facility or rehabilitation setting preferred (new grads welcome to apply) Strong interpersonal, communication, and time-management skills, Commitment to providing compassionate, evidence-based patient care.
