Job Description:
Encompass Health is seeking a dedicated Case Manager to join our
innovative healthcare team. This role requires a compassionate and
knowledgeable professional to oversee the integrated care plans of
patients in our facilities spanning 42 states. The successful
candidate will be responsible for coordinating with healthcare
providers, patients, and families to ensure high-quality,
patient-centered care following our model of facility-based and
home-based services. Through effective case management, the Case
Manager will facilitate individualized care that promotes recovery,
health, and well-being. Assess new patients, establish care plans,
and monitor progress Collaborate with healthcare professionals to
coordinate patient care Advocate for patient needs and serve as a
liaison between patients and healthcare providers Monitor patient
outcomes, adjusting care plans as needed to achieve optimal results
Ensure compliance with healthcare regulations and standards Provide
education to patients and families about healthcare options and
resources Document patient care activities and participate in care
conferences Strong interpersonal and communication skills
Proficient in healthcare management systems and electronic health
records Ability to work collaboratively in a multidisciplinary team
Critical thinking and problem-solving skills Knowledge of
healthcare regulations and case management practices Medical
Insurance Dental Insurance Vision Insurance 401k Certified Case
Manager (CCM) preferred Licensed Registered Nurse (RN) or Licensed
Social Worker (LSW), depending on educational background
