Job Description:

Genesis HealthCare is seeking a compassionate and skilled Social Worker to join our dedicated team, focusing on providing the highest standard of care at our rehabilitation and long-term care facilities. In this role, you will work closely with patients coming out of acute hospital settings to help them transition smoothly to lower levels of care, and to manage their long-term health needs effectively. You will be tasked with developing care plans, coordinating with healthcare professionals, and providing emotional support to patients and their families, aligning with our commitment to clinical excellence and patient well-being. Assess patients' social, emotional, environmental, financial, and support needs Develop comprehensive care plans in conjunction with healthcare teams Facilitate patient access to community resources Support patients and their families through counseling and assistance with healthcare decisions Coordinate with healthcare providers to ensure continuity of care Maintain accurate records of patient interactions, services provided, and outcomes Empathy and strong interpersonal skills Excellent communication and organizational skills Ability to work collaboratively in an interdisciplinary team Problem-solving and crisis intervention abilities Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed Social Worker (LSW) required, with LCSW preferred.
