Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or relevant accrediting body. Current, unencumbered New Hampshire physical therapist license or active license for the state the clinician will practice in prior to start. BLS/CPR certification required. Experience providing home health physical therapy and conducting home visits. Must have reliable transportation and valid driver’s license. Familiarity with electronic documentation and basic computer skills, including email and online training platforms. Client-preferred: 2–3 years of physical therapy experience. Assignment Details Facility type: Home Health. Shift: Days. Schedule format: 5x8. Must supervise and evaluate PTAs and home health aides as assigned. Must perform patient evaluations, develop and update plans of care, document condition and progress, and provide in-home treatment per physician orders.
