# # Job Summary: The Behavioral Health Care Manager is a member of the Community Health Team (CHT); working collaboratively with the Community Health Team Leader, Community Health Workers, Interns and other team members to support the behavioral health needs of the patients we serve, under the direction of the department manager. This position and the other team members function as extensions of the primary care practices from who we receive referrals, working collaboratively as part of a multidisciplinary team within the practices and the surrounding community. The primary contacts for the BHCM are the Nurse Care Managers of the primary care practices, case managers within the hospital, home health colleagues and any potential behavioral health workers within the practice/community. The BHCM provides home, community and office based behavioral health screenings, assessment, referral and care coordination for medically complex patients who require behavioral health services, including mental health and substance use. In addition, this position provides short term supportive counseling utilizing evidence based strategies, provides patient education, supports treatment adherence, and takes action when non-adherence occurs or symptoms worsen using motivational and health coaching techniques as indicated. This position acts as a liaison between CHT patients and external behavioral health resources, supporting care plan needs. Reports to: Manager, Community Health Team Minimum Qualifications: # Masters degree in Social Work, Marriage and Family Counseling or Mental Health Counselor. Licensed according to discipline in the State of Rhode Island. Eligible for credentialing by insurance carriers. # Minimum of two years direct clinical experience working with adult and/or families providing mental health and substance use counseling # Three to five years of experience working in at least one of the following areas: community health setting, integrated behavioral health, acute care services or chronic care management program. # # Experience working with primary care providers coordinating care and providing care management services across the healthcare continuum required. # The ability to travel to various locations in the state, typically via car is required. # Proficiency in the use of computers, including but not limited to Microsoft Office Suite and EMR software . Able to assess and differentiate priorities # Excellent interpersonal skill # Excellent written and verbal communication skills # Able to maintain confidentiality with all aspects of information (including patient data) in accordance with the program#s philosophy and policy, and state and federal regulations. Must handle the most sensitive and confidential matters with the utmost discretion. # Proficiency with computer skills (i.e., Microsoft Word, Excel and Access, and Web-based applications).
