Community Transition Facilitator

The Facilitator works as a member of a team but reports to the
Director of Regional Transition
--- Analyze job tasks and work environments so as to plan
appropriate training, supervision and support services for clients
placed in assessment or work development activities
--- Collect referral information and authorizations and create case
file. Request client records and necessary paperwork to proceed
--- Coordinate community assessment placements with school, family,
and business partners.
--- Provide training and instruction that will enable clients to
learn job tasks and integrate naturally into the business work
--- Observe and record client performance and work behaviors and
complete required assessment reports.
--- Liaison with business personnel regarding: support needs of
clients placed; client's work performance; and building natural
--- Develop and maintain relationships with business partners.
--- Maintain ongoing communication with Vocational Services
Coordinator regarding client evaluation or training process and
--- Maintain ongoing communication with ORS Counselor, school
personnel and family regarding progress of client.
--- Attend Intake, IEP/IPE, Assessment Review meetings as
--- Work with other Project staff in meeting the overall goals of
--- Submit all required reports in a timely manner.
--- Other tasks as assigned.
Minimum Associates degree in human services, Bachelors degree
preferred. Experience with adolescents; knowledge of developmental,
mental health, physical, and learning disabilities. Excellent
communication skills and writing skills. Must be available to work
school and non-school hours and have a safe driving record and be
willing to transport students and clients in agency vehicle.
