The ideal candidate will be results-driven, team-and
detail-oriented professional who is comfortable working on multiple
tasks in a deadline-oriented environment. Responsibilities and
Duties:
- Provide assessment, coordination, advocacy, and support to the
clients of the CSP program for the purpose of creation,
development, or enhancement of an individual support network.
- Provide internal and external training related to the value and
necessity of support networks in the maintenance of positive mental
health.
- Assess the volume, reciprocity, and positive value of
individual support networks.
- Develop and implement treatment plans and interventions aimed
at improving the support networks of assigned caseload or
individual.
- Develop contacts and relationships with collateral community
agencies or organizations that may assist in the fulfillment of
support network functions.
- Teach basic communication skills that would enable an
individual to succeed in developing positive relationships with
others.
- Produce billing forms, logs, data collection, and clinical
documentation in accordance with Organization standards and State
and Federal licensing regulations. Qualifications and Skills:
- Master's degree in Rehabilitation or related field or
certificate as a career development facilitator, or
- A Bachelor's degree in the human services, business, or related
field
- Valid driver's license required
- Supported Employment Professional Certification preferred, or
