• Provide assessment, coordination, advocacy, and support to the clients of the CSP program for the purpose of creation, development, or enhancement of an individual support network.
• Provide internal and external training related to the value and necessity of support networks in the maintenance of positive mental health.
• Assess the volume, reciprocity, and positive value of individual support networks.
• Develop and implement treatment plans and interventions aimed at improving the support networks of assigned caseload or individual.
• Develop contacts and relationships with collateral community agencies or organizations that may assist in the fulfillment of support network functions.
• Teach basic communication skills that would enable an individual to succeed in developing positive relationships with others.
• Produce billing forms, logs, data collection, and clinical documentation in accordance with Organization standards and State and Federal licensing regulations. Qualifications and Skills:
  • Master's degree in Rehabilitation or related field or certificate as a career development facilitator, or
  • A Bachelor's degree in the human services, business, or related field
  • Valid driver's license required
